If you are covered under one of the above companies and your group plan allows Direct billing you will not have to pay up front for your treatments.
If you have a benefit plan not listed above yet, you are eligible to submit your invoice to them for reimbursement - if you have coverage for Paramedicals.
We will determine how much your plan covers for your treatments, and you will be responsible to pay any costs not covered by your insurance plan.
Note: If you are covered by a company that does not allow direct billing, you will be responsible to pay for your treatments in full, and then submit your receipts yourself for reimbursement.
Motor Vehicle Collision Insurance (MVA)
Have you been injured in a motor vehicle collision? If so, there are a few steps to follow:
- Report the accident to your insurance. You will be given a name of a bodily injury adjustor and a claim number we will need this information to ensure all your documentation is sent to the right person.
- See your family physician. They can rule out severe injuries and prescribe appropriate medications.
- Arrange with our office an appointment for an assessment – ensure it is within 10 days of your accident or you may not be approved for the initial 21 in protocol treatments.
- You will be required to fill out an AB-1 form which details what happened in the accident and then the chiropractor will fill out the AB-2 form detailing a treatment plan to the Insurance company. These are available from our office. Your insurance will authorize a certain number of treatments based on the injuries that you sustained up to the maximum of 21. Your Chiropractor and the office staff will assist you in the authorization process.
Workers Compensation Board (WCB)
- If you have been injured at work take steps to make a claim in a timely manner. Advise your employer and ensure that a claim is filed with the Worker’s Compensation Board (WCB). Both you and your employer will have to fill out information for WCB.
- You must see a family physician who will also file your WCB paperwork after completing a medical exam.
- Your first assessment and treatment by Castledowns Medical & Wellness Centre are covered by WCB. We will need authorization from WCB for further treatments.
